Fahe Member & Affiliated Partner Tools for Hurricane Helene
Although we are not first responders, we can prepare now for connecting residents to programs that will help them to recover from Hurricane Helene.

Fahe recommends that Members and our affiliated partners begin capturing resident data about how the hurricane has impacted them through an intake form that we have created. By collecting data starting now, we can track individual needs and develop an aggregate picture of the impact of the disaster and what might change over time.
Needs Assessment Form / Guidance
Fahe provides a Needs Assessment form, which you can use in the field on a device with internet access.
By submitting this form, you give permission to share information requested in this survey with Fahe, its Members, & its Federal, Regional, State, Local and other partners. The survivor understands the purpose of this survey is to create a database of Disaster Recovery survivors needs for assessment and use in future potential programs.
